In collaboration with the multi-disciplinary renal team, the CKD educator provides patients & families with high quality evidence-based information about chronic kidney disease, treatment options, risks & benefits, and supportive care.
Information is tailored to both person & family, culture, language, and is relevant to the individual’s social & psychological needs.
Strategies will focus on enabling patients & families to be self-caring and independent where possible as well as to make informed, shared decisions in a timely fashion, before treatment is required and at any time during their CKD journey.
